Military Discharge DD214 Request For Certified Copies

Who May Obtain a Copy of Records of Veteran who separated from service at least 62 years ago:

  • A Military Veteran
  • With the Military Veteran’s written permission (or if Veteran is deceased, proof of death), a Military Veteran’s Next of Kin

The next of kin can be any of the following:

  • Surviving spouse that has not remarried
  • Father
  • Mother
  • Son
  • Daughter
  • Sister
  • Brother

OR

  • With the Military Veteran’s written permission (or if Veteran is deceased, proof of death), the Montgomery County Veterans’ Assistance Commission (VAC) Officer or a Representative of the Department of Veterans’ Affairs

Fees

There is no charge for certified copies issued.

 

What You Need

1.) You MUST fill out a Military Discharge request form
2.) Submit the form by Mail or In-Person

Archival Military Discharge DD214 Request For Non-Certified Copies

Who May Obtain a Copy of Records of Veteran who separated from service more than 62 years ago:

  • A Military Veteran
  • With the Military Veteran’s written permission (or if Veteran is deceased, proof of death), a member of the public

Fees

There is a 50 cents per page charge for Non-Certified Copies.

 

What You Need

1.) You MUST fill out an Archival Military Discharge request form
2.) Submit the form by Mail or In-Person

 

Request forms cannot be faxed or emailed.
